COMPLETION OF THIS FORM CONSTITUTES AN AGREEMENT THAT THE SCHOOL WISHES THE GOLDEN THREAD TEACHING SCHOOL HUB TO ACT AS THE APPROPRIATE BODY FOR THE NAMED ECT. YOU WILL NEED TO SUBMIT THE ASSESSMENT FORMS THROUGH THE PORTAL BY THE DATES STATED ONCE THE ECT IS REGISTERED.
